In consideration of being allowed to participate in any way in the try outs conducted by the Fort Wayne United Soccer Association and related events and activities, the undersigned hereby agrees that the parent(s) or legal guardian(s) will instruct the minor participant that, prior to participation, he/she should inspect the facilities and equipment to be used and, if the participation believes anything is unsafe, he/she should immediately advice a representative of the association of such condition(s) and refuse to participate. The undersigned acknowledges and fully understand that the minor child will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inactions or negligence, but the action, inaction or negligence of others, the rules of play, or the condition of the premises or of any equipment used. The undersigned further understands that there may be other risks not known to the association or not reasonably foreseeable at the time. The undersigned assumes all the foregoing risk and accepts personal responsibility for the damages following such injury, permanent disability or death. The undersigned herby releases, waives, discharges and agrees not to sue Fort Wayne United Soccer Association, its affiliate clubs, their respective administrators, directors, agents, coaches, volunteers, or employees of the organization, other participants, sponsoring agencies, the undersigned, his or her heirs and next of kin, for any and all claims, demands, losses or damages on account of injury, including death or damage to property caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.
I, the parent or guardian, understand that Citadel Futbol Club does not carry insurance covering my child’s participation. I accept financial responsibility for any injury which may occur resulting from my child’s participation. Furthermore, I agree to hold harmless and indemnify the directors, coaches, referees, or any assigned officers against any and all litigation that may arise out of my child’s participation in this soccer program.
