Procedures for Fall 2008

3. Coaches are requested to make 2 copies of registrations for each player; submit the original to the Registrar (John Fyfe). Each registration form must contain the coach's name, as well at the other required information. Players new to FWUSA, and those who have not been active for two years must submit a copy of birth certificate. Please mark new players on registration form.
   
   
4. Each registration must be accompanied by a check for $65.00, made out to FWUSA. It is helpful if the player's name is on the check. Do not staple the check to the registration form; a paper clip is fine. It is extremely helpful if you can put the registrations in alphabetical order before submitting them.
   
   
5. Team registrations are due IMMEDIATELY for Fall '08. As soon as you have the minimum number of players for your roster (7 for small sided, 10 for full sided) please submit immediately, so we can register the team.
   
   
6. Please have all registrations submitted to you, for your records; then submit the registrations, birth certificates and fees to the Registrar (see below).
